If you purchased any Extended Services Plans from Staples in New York State from February 1, 2010 to August 31, 2013, you may file a Claim Form to receive a payment from this Settlement. Claim Forms must be submitted online via this website or mailed to the Settlement Administrator no later than February 26, 2018. Claim Forms filed after this date will not be considered timely and may not be accepted.
This Settlement ONLY applies to Staples in New York State. Do not file a Claim Form if you did not purchase an ESP from a Staples in New York State.
After you click the button below, you will be asked to provide your contact information, and information about the ESP and the product you purchased it for. If you have any receipts, you will also be able to upload copies of them. Please have all this information ready before you start your online Claim Form. You will not be able to save your progress and come back at a later time to finish. Claim Forms must be submitted online by February 26, 2018 at 11:59 PST.
If you would prefer to file a paper Claim Form, download a copy here. You may also request a Claim Form by emailing info@StaplesSettlement.com, calling 1-888-457-2303, or writing to the Staples ESP Settlement Administrator at the address below. Please read the instructions carefully, fill it out to the best of your ability, and attach any copies of receipts you would like to include. Then mail it to the Settlement Administrator so that it is postmarked no later than February 26, 2018.
